MAKE DECLARATION VIA TOKHAIYTE.VN

Access https://tokhaiyte.vn or download the app “Vietnam Health Declarations” on your mobile phone. Choose “For domestic move declaration” and fill in the form.

After submitting the form, please:

• Print/capture the QR code to confirm that the medical declaration has been completed.

• Present at the departure and arrival airport (if required).

MAKE DECLARATION VIA APP NCOVI OF MINISTRY OF HEALTH

Download NCOVI on your mobile phone on App Store/CH Play.

Open the app, register and fill in the declaration form.

Keep the QR code on the app for presenting at the airport (if required). Noted that the QR code is only valid in 5 minutes.

MAKE DECLARATION AT THE AIRPORT

The paper declaration form at the aiport is the same as the form on website and app in Method 1 and 2.

Please fill in the form and hand it back to the airport staff.
